Case management works to eliminate common barriers for those struggling with mental health issues. Coleman case managers act as safety nets, helping clients deal with life’s daily stressors and unexpected, overwhelming events.

Coleman case managers work with a collaborative network of therapists, school staff, community partners, and other mental health and primary care professionals to provide holistic, whole-person care. This may include anything from coordinating doctor visits to providing education around coping skills.

Who is case management for?

Coleman case managers support all clients and families experiencing mental health issues. Clients who benefit from case management services are typically those who experience severe and persistent mental health issues and may also be struggling with finances and/or housing. At Coleman, these clients can receive treatment in individual and/or small group sessions for ongoing assessment of needs, medication management, and living and coping skills development.
