Our intake process is designed to screen for safety and connect our clients with the right services for their needs.

About Assessment

An assessment is a clinical intake provided by a licensed professional. The process consists of gathering information about the initial problem, evaluating the client’s current mental status and needs, and beginning the treatment planning process. This helps clinicians determine the best mix of mental health, addiction, and/or employment/vocational rehabilitation services to assist in recovery. 

No referral is needed to complete your assessment and begin services with Coleman. Below is a list of requirements you will need to bring with you to your assessment session:

  • Insurance card
  • Photo ID
  • Social Security number
  • Proof of income (W-2 or pay stubs)
  • List of current medications and contact information for current health providers
  • Custody or guardianship paperwork if applicable 

If you do not have insurance or adequate income, we will connect you with resources to determine eligibility for assistance and our sliding fee scale.

How We Evaluate

When seeking an assessment at Coleman Health Services, expect to discuss the following with the clinician:

  • Personal history
  • Initial symptoms
  • Diagnostic impression and treatment recommendations
  • Use of alcohol and/or drugs
  • Home environment/relationships with family and peers
  • Current mental status
  • Leisure and recreational activities
  • Childhood history
  • Military history
  • Financial status
  • Usual social and peer group activities
  • Family circumstances (including custody issues)
  • Vocational/educational assessment
  • Legal assessment
  • Nutritional and health status
  • Spirituality
  • Concerns of safety of self or others
  • Strengths

At Coleman Health Services, we strive to provide the fastest access to care, and one way we do that is through our assessment walk-in clinics. This allows Coleman to ensure your safety, provide support, and identify the referrals needed to begin recovery.
