Warren, Ohio (November 11, 2025) – Coleman Health Services announced today it will relocate its Trumbull County behavioral health services to the newly developed facility at 820 Pine Avenue SE in Warren—the same site that houses the new Behavioral Health Crisis Center and Sister Jean’s Lighthouse, a supportive housing project for women and children. The move, taking place November 25th, represents a major step forward in expanding access to comprehensive, person-centered behavioral health care in the region.
The 17,080-square-foot facility—plus an additional 7,670 square feet of renovated space—will become a centralized hub for mental health, substance use, and crisis intervention services. The location was developed in partnership with the Trumbull County Mental Health and Recovery Board and other community collaborators.
“By moving our behavioral health programs to this new facility, we are bringing together the full continuum of care—outpatient, crisis, and supportive housing—under one roof,” said Hattie Tracy, President and CEO of Coleman Health Services. “This integrated model will allow us to better serve the community and meet people where they are in their recovery journey.”
Enhanced Access and Integrated Care
Once open, Coleman’s Warren office will provide expanded access to outpatient therapy, psychiatry, case management, substance use disorder treatment, and crisis stabilization. The facility’s design supports efficient coordination among service teams and offers a trauma-informed, welcoming environment for individuals and families in need.
The Crisis Access Center, operated by Coleman, will feature private intake rooms, counseling and case management offices, MRSS (Mobile Response and Stabilization Services), and on-site pharmaceutical services. A secure entrance and dedicated ambulance parking will ensure safe and timely access for emergency personnel.
A Collaborative Community Investment
The facility and its supportive housing component, Sister Jean’s Lighthouse, are supported by a mix of local, state, and federal partners—including funding from the U.S. Department of the Treasury, OhioMHAS, and the City of Warren.
