Case Management

Case Management is a community-based service provided by an individual or a multidisciplinary team. Coleman Behavioral Health provides case management social work to individuals based on their optimum level of care. Individuals are referred to case management services from a behavioral health assessment completed by a licensed professional.

Case management activities focus on:

  • An ongoing assessment of needs
  • Assistance in achieving personal independence in managing basic needs as identified by the individual and/or parent or guardian
  • Further development of daily living skills
  • Assistance with accessing natural support systems in the community
  • Developing an individualized treatment plan
  • Connections to formal community supports
  • Behavioral health symptom monitoring
  • Crisis management
  • Advocacy and outreach
  • Individual and family education and training
  • Eliminating barriers to seeking or maintaining education and employment

The amount of time in service is individualized to meet the needs of each person’s recovery process.
