Position Description

Coleman Professional Services (CPS) is a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs to foster recovery, build independence, and change destinies for individuals, families, and businesses in our community.  We are seeking social work or counseling individuals to be outpatient therapists in Portage County. We are in need of a Therapist (Substance Use Disorder Specialist) and also a Therapist (Diagnostic Assessment Specialist). Both jobs are full time and Monday – Friday first shift. 

Substance Use Disorder (SUD) Specialist provides mental health and addiction services (SUD) for adults. Minimal requirements include counselor trainee and CDC-A; a NPI number; an Ohio Medicaid number; and two years mental health experience.  PCC and LICDC or PC and LCDC II or III is preferred (must have a NPI number; an Ohio Medicaid number; and a CAQH account). Also, preferred are independently licensed providers with SUD expertise who can bill Medicare. 

Diagnostic Assessment (DA) Specialist provides initial Diagnostic Assessments, post-hospital assessments and counseling services for adults.  Minimal requirements include licensed as LSW or PC with a Master’s degree; a NPI number; an Ohio Medicaid number; and have community mental health experience.  Prefer PCC or LISW.
