FACT Program Navigator/Case Manager (SUD) - FT - Allen County

Position Description

Individual will support and engage individuals participating in the community collaborative, FACT program. The navigator will work with individuals who have been accepted into the program with a child support need to identify barriers, and develop and monitor person centered treatment plan. The Navigator is responsible for the following: 

  • Create the Individualized Treatment Plan and remove any/all barriers so that the participants can accomplish set goals
  • Facilitate communication with Coleman service providers and Child Support Enforcement Agency
  • Build partnerships with community organizations to improve integration behavioral health and other social service organizations
  • Link individual to services if necessary
  • Conduct case reviews
  • Track results and monitor outcomes data

This individual will also be a team player for our case management program.  Responsibilities include:                            

  • Establish client outreach plans
  • Provide support in development and accomplishment of goals for individuals with substance use disorders
  • Establish community/client relationships
  • Assist clients with daily living skills and following their Individual Service Plans

Qualified candidates will an Associates in Human Services or related field and able to work a flexible schedule to meet the needs of our consumers. Bachelor’s degree in criminal justice, psychology or related field preferred.
