Community Care Navigator - FT - Auglaize CountyLocation: AuglaizeType: FTPosition DescriptionSeeking an individual to work with the Auglaize county Police Department, Auglaize Fire Department and referring community partners to assist clients within the community in efforts to identify service needs. Contributes to patient’s comprehensive transitional aftercare priorities. Provide expedited entry into Coleman services by offering diagnostic assessment, SBIRTS, and brief interventions within the community stations. Assist Auglaize Police Department and Auglaize Fire Department on calls for clients in the midst of a crisis to determine if EMC and assessment for hospitalization is necessary. This position will also be available to local community partners for consultation and emergency responses as needed. Minimum requirements are an LSW/ LPC and 2 years crisis intervention experience, have a NPI number and an Ohio Medicaid Number. Masters prepared/ Independently license preferred.
