Case Manager/Social Worker (SUD) - FT - Mahoning CountyLocation: Trumbull,MahoningType: Position DescriptionColeman Professional Services, a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs, is expanding services to Mahoning County and seeks a team player for our case management program to work with the Substance Use Disorder population. Responsibilities include establishing client outreach plans, providing support in development and accomplishment of goals, establishing community/client relationships, and assisting clients with daily living skills, medication monitoring, and following their Individual Service Plans. This position generally must spend a majority of its billable time in the community and transport clients. Qualified candidates must have an understanding of: mental illness, psychiatric symptoms, and impact on functioning and behavior; how to therapeutically engage a mentally ill person; concepts of recovery/resiliency; crisis response procedures; community mental health system; de-escalation techniques; how his/her behavior can impact the behavior of individuals with mental illness. At least one year related experience and knowledge of community resources is required. Qualified candidates must also have experience working with dually diagnosed individuals and must have and maintain a valid driver's license and maintain acceptable driver status in accordance with Company policy. Must be willing to work flexible schedule to meet the needs of our consumers and transport clients. CDCA required. LCDCII or higher preferred.
