Case Manager/Outreach Coordinator - FT - Jefferson CountyLocation: JeffersonType: ftPosition DescriptionSeeking an individual to provide intensive, case management services to individuals with severe and persistent mental health issues in Jefferson County. Establish supportive relationships with other community operations, such as soup kitchens, drop in centers, etc. Coordinate trips to various community events in efforts to connect the client in their community. Assist in maximizing independent community skills and demonstrating appropriate home living skills. Represents the program in a positive and professional manner, both in the workplace and in the community. In addition, this person will work closely with State and Federal regulations and databases. Qualified candidates must be familiar with HUD guidelines and standards, have BA/BS in mental health or related field or five years experience in mental health or related field. Must have and maintain excellent driving record while also be willing to drive out of the county. Experience in case management preferred.
