Case Manager (Mobile Response Youth) - FT - Stark County

Position Description

Coleman Professional Services, a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs is seeking case manager who will work in collaboration with Mobile Response Counselors to foster rapid child and family stabilization. Emphasis is placed on imminent risk reduction, increased coping skills and function, diversion from emergency departments, inpatient settings, and community retention. Duties include: 

  • Provide short-term, community-based case management to assigned families
  • Provide related functions to aid and support improvement in independent functioning within the community
  • Coordinate and participate in the development of service plans
  • Provide ongoing assessment of needs, support, service navigation and linkage with appropriate resources and services
  • Function as part of an interdisciplinary team and develop and maintain effective relationship with community professionals

Qualified candidates will have their Bachelor’s degree in a related field or three years related experience (includes internships and volunteer experience); have and maintain an excellent driving record; and familiar with child development, family systems, trauma informed care, community resources, motivational interviewing, crisis intervention, and case management approaches. Prefer someone with prior case management experience.
